2. Do not submit patient information
The public website forms are not designed for protected health information. Do not submit patient names, dates of birth, medical record numbers, diagnoses, test results, prescription details or other patient-identifying information through these forms.
If patient information is submitted unintentionally, BioRelay may restrict access, delete the information when legally permitted, or contact the sender to arrange a more appropriate method. - Protect the website, prevent fraud or misuse, troubleshoot problems and comply with legal obligations.
- Send marketing communications only when permitted. Recipients may opt out at any time.
4. How we disclose information
